Please write down the name, product number, the amount and sizes (cuffs only) of products you want to order as well as your complete
name and address. If you like this order to be delivered to another recepiant (as a gift maybe) different from your address, please write
that address with the note "send as gift to" after yours down.

You'll get an order confiramtion with needed payment information as soon as possible. All payments have to to be prepaid.
After the payment is done and confirmed by my bank i'll send your order to the given address.
